Recurrent angina after coronary artery bypass grafting (CABG) or percutaneous coronary intervention (PCI) may indicate the development of acute complications, new lesions, stent thrombosis, or restenosis. Chest pain up to 48 hours after percutaneous intervention suggests the occurrence of acute obstruction, transient coronary spasm, nonocclusive thrombus, branch occlusion, or distal embolization. Recurrent chest pain approximately 6 months after bare-metal stent implantation or, later, after drug-eluting stent implantation is most likely related to restenosis. On the other hand, the onset of angina after 1 year of stent implantation is usually related to new coronary lesion or stent restenosis due to neoatherosclerosis.
